Owen Sound is located on Georgian Bay, on the Bruce Trail, servicing a regional role to 150,000 people. Both summer and winter vacationers contribute to the hospital emergency experience. Family physicians remain as most responsible physician except in CCU/ICU/Surgery. There is a good specialty support for our role in general.

Most significant procedures are done through ambulatory care at the hospital (excisions, cast applications, flexible sigmoid). Three full time emergency physicians, two of who are former community family physicians and a declining number of community family physicians run an extremely busy emergency department. They have, to date, expressed an interest in allowing our residents an opportunity to work with them as time permits.

 

Excellent home care services and an active public health unit facilitate the family physician=s community role.

 

Most physicians have made a life style decision to avoid big city life and are active sailors, outdoor lovers and hobby farmers. Owen Sound offers many cultural activities such as community orchestra, renowned Little theatre, and summer folk festival.

Although in a separate building, the clinic is located on the hospital grounds, and therefore a convenient location for participation in in-hospital activities and emergency work. Patient population includes a few Old Order Mennonite families which offers the opportunity for experience in dealing with a patient population who choose not to have provincial health care coverage.

 

Doctors have an extensive involvement in patient work up and admissions plus ongoing hospital care.

 

There is a high volume of office patients of all age groups. Visits to the 2 Nursing Homes in Chesley are a regular part of practice activities. Excellent example of a group practice where physicians work well together.

 

Dr. Sanborn practices a large amount of internal medicine and is interested in women=s health issues and contraception. She is involved with the Sexual Health Clinics at the local health unit. Dr. Thompson can arrange for residents to spend time at the Sexual Health Clinic in Owen Sound.

 

While in Chesley, residents have the option to stay in the home of one of the clinic physicians.

Geriatrics: There is a 30-bed geriatric wing at the Port Colborne General Hospital and a 90bed Regional Nursing Home in Port Colborne. There is also a 100-bed retirement home which deals with a fairly frail population

 

Office Procedures: Minor surgery done in the office along with cryotherapy, urinalysis, and hemoglobins

 

Surgical Assists: Surgical assisting is available in orthopedics, gynecology, general surgery and urology

 

Palliative Care: Home palliative care is supervised by each physician with good Home Care resources as well as a Community Palliative Care Program initiated by the Port Colborne General Hospital. Trained palliative care volunteers through this program are available for care in the patient=s home or in the hospital, local retirement home, or local nursing home. Palliative Care services are provided for in patients at the Port Colborne General Hospital.

SETTING: The community of Port Colborne, a municipality within South Niagara, is located on the north shore of lake Erie and has been designated a rural community. The population of Port Colborne and catchment area is 27,000 with 17% of the residents being over age 65. There is a large seasonal population particularly in the summer months. It is estimated that approximately 500,000 people visit Port Colborne annually.

 

The leading causes for medical interventions are heart disease, digestive disease, respiratory disease, cerebrovascular disease, breast cancer, and lung cancer.

 

The community of Port Colborne is in close proximity to tertiary care services in Hamilton, Toronto, London and Buffalo. Medical Specialists provide consultative services as well as out patient clinic services. Primary Care Services in Port Colborne are provided by 12 family physicians comprising of 3 group practices and 3 single practices. Based on a planning standard of one family physician per 1,400 population, there is a need for 19 full-time physicians in the community of Port Colborne. A comprehensive community health needs analysis recently completed indicates a need for physicians with an interest in family practice, mental health, anaesthesia, general surgery, palliative care, emergency services, and geriatrics.

 

The Port Colborne General Hospital and community of Port Colborne have developed incentive packages for interested residents to assist with costs related to relocation, start-up, and education. A variety of practice options as well as locum arrangements are also available.

EDUCATIONAL OPPORTUNITIES:

 

A busy solo family physician in Hagersville since 1981, a town of 2,500 people with a service population of 16,000, within 20 miles of Brantford and Simcoe. In town there is a fully accredited 42-bed hospital offering a wide variety of services to the surrounding community including a 24-hour emergency room. Dr. Doroszkiewicz= clinic is located 1/2 mile from the hospital. The clinic has been designed for 2 physicians and has a lab on site. There is an excellent patient mix and a very pleasant community. The local hospital has specialty and sub-specialty outpatient clinics that operate on site. Also in close proximity to major hospitals in Hamilton and use these as their tertiary referral centres.

The population of Port Colborne and its catchment area is 27,000, with 17% of the residents being over age 65. There is a large seasonal population in the summer months. It is estimated that approximately 500,000 people visit Port Colborne annually.

 

Port Colborne is in close proximity to secondary services in Welland, St. Catharines and Niagara Falls. Primary care services in Port Colborne are provided by 12 family physicians comprising 3 group practices and 3 solo practices. A comprehensive community health needs assessment recently completed indicates the need for 19 full-time family doctors with an interest in mental health, anesthesia, palliative care, general surgery, emergency services and geriatrics. The Port Colborne General Hospital has 30 acute beds, 6 intensive care beds, and 30 chronic care beds. The fully equipped 24-hour Emergency Department sees approximately 17,000 visits annually.

 

The Port Colborne General Hospital and community of Port Colborne have developed incentive packages for interested residents to assist with costs related to relocation, start-up, and further education. SETTING: Goderich provides the opportunity for Family Practice residents to have an extensive rural family practice training experience. Goderich, population 7500, is a resort town on Lake Huron with an extensive variety of summer and winter recreational opportunities, including many water sports and fishing. It has an industrial base (salt mine, motor grader manufacturing) and a surrounding rural agriculture area.

 

The resident has the opportunity to be directly involved in the full range of family practice, including the ongoing and continuing care of office patients as well as managing those patients when they become ill and require hospitalization. Typical for rural family practice, out practice involves extensive hospital work including obstetrics, emergency department work and direct in-hospital care of patients of all kinds. Nursing home visits and house calls are also an important and enjoyable part of our practice. Teaching and supervision will primarily be done by Drs. James and Leslie Rourke. There will be an opportunity for residents to be exposed to female and male physician role models, both in this practice and others in town. Colleagues are supportive of Drs. James and Leslie Rourke=s teaching and are interested in providing some teaching and supervision but on a smaller scale. This provides exposure to a variety of different practice styles. They are in a group coverage with their colleagues for obstetrics, emergency medical coverage of their practices and in-patient coverage. The doctors are on call approximately 1 weekend per month for obstetrics and hospital patients and 1 night per week for the Emergency Department. Patient care based learning will be emphasized along with a rural practice curriculum which includes extensive reading relevant to rural family practice and family practice in general.

 

In 1991 and 1992 Dr. James Rourke was awarded ACommunity Teacher of the Year@ by the Ontario College of Family Physicians. SETTING: The local health clinic auxiliary has completed a 3-bedroom home adjoined to the clinic which is used for rental accommodation for residents and/or medical students while in Tobermory. The home has basic furnishings but residents should bring their own towels and bedding. There is no washer or dryer but there are 2 Laundromats in town.

 

Tobermory is a small town located 180 miles north of Hamilton at the tip of the Bruce Peninsula. It is known for its excellent diving, beautiful coastline and for the Ferry linking it to Manitoulin Island and the TransCanada Highway. The permanent population in town is about 300, but the population served by the clinic is about 3000.

 

The clinic at Tobermory is extraordinarily equipped. It includes a large waiting area, 2 examining rooms, a small surgery with general anaesthesia available, an eye room, X-ray facilities, a small laboratory, an attached hyperbaric facility. The hyperbaric tank is a walk-in chamber used for treating diving accidents and approved medical/surgical conditions.

The clinic is staffed by 4 full-time family physicians with the occasional addition of a physician on locum during the busy summer months. There are also 2 receptionists and a registered nurse on staff. A number of consultative services are provided by the physicians on staff. Dr. Suke performs allergy testing and asthma assessment as well as cardiac stress testing.

 

There is a satellite clinic in Lion’s Head, 30 miles to the south. This clinic is annexed to a small 10-bed hospital serving the local area. The Lion’s Head Clinic has a large waiting area and 3 examining rooms. The adjacent hospital is staffed by the local family practitioners. It includes an out-patient department and obstetrical facilities, plus X-ray. There are no consultative services or laboratory facilities and patients requiring such services are normally referred south.

 

The family physicians in Tobermory perform all their own minor procedures, and inpatient management. They also act as consultants in hyperbaric medicine and frequently perform their own X-rays and laboratory tests.

 

The summer months are extremely busy. The winter months are much slower, but a steady pace is maintained.

(SOAP format) and stored electronically. Consultant notes, X-ray reports, etc. are scanned into the electronic record. Each chart has a very detailed cumulative patient

patient profile (typed) which is continuously maintained current. All lab data downloads

automatically by modem in to the electronic chart. Detailed searches can easily be done

(ie, all men with a previous myocardial infarction who are not on entrophen or all diabetics who are on digoxin and a diuretic but not on a potassium supplement)
