Gadson TM Corp Computer Support Survey

This document is an online survey. Using a form and a CGI script, you can ask your staff to fill out a questionnaire, asking them their opinions on a number of things. For MIS, these questions could focus on things like what kind of software employees enjoy or dislike using, what kind of hardware/software combinations seem prone to crashing, and so on. Be sure to include a field in your form for email addresses and employee info.
