 I've included this here because just simply, it is such a beautiful song.

 The only real chords to this song are a D and a G. Play the G higher than the D. You can play
 them anywhere you like. I mix it around just for variety. Sometimes I'll play them as bar
 chords, or open chords, or I might even play the G as an open D shape on the 7th fret.

 At the chorus it an E is added.

 There is this tiny, but sweet riff at the very end:
