His father, David, contracted a terrible, fatal illness with no known cure. Instead of watching his father suffer any more to await a lingering death, he mercifully pulled the plug at his father's request. However, not long after that incident, a cure for his father's disease was found and he has carried the guilt of his father's possibly needless death with him for years.

McCoy suffered a serious overdose of cordrazine in a shipboard accident. He fled the ship and jumped through a time portal being studied by the Enterprise at the time. In the past, McCoy was seriously affecting the time line, so Kirk and Spock had to follow him to restore the shape of history.

He contracted the terminal 'xenopolycymethia' and retired from Starfleet to spend remaining days on asteroid ship Yonada and soon married that worlds Hight Priestess, Natira. When Spock found a cure in the Yonada's computers, he left her to return to the service.
