Just prior to entering the dilithium crystal chamber, which he knew would kill him, Spock placed his 'katra' in the mind of McCoy which nearly drove him crazy at least to the point of Starfleet detention. Spock's katra was removed from him in the traditional Vulcan refusion process once Spock's body was recovered.

His father, David, contracted a terrible, fatal illness with no known cure. Instead of watching his father suffer any more to await a lingering death, he mercifully pulled the plug at his father's request. However, not long after that incident, a cure for his father's disease was found and he has carried the guilt of his father's possibly needless death with him for years.
As a Lt. Cmdr., he was named CMO on board the Enterprise under James T. Kirk, replacing Mark Piper.
McCoy suffered a serious overdose of cordrazine in a shipboard accident. He fled the ship and jumped through a time portal being studied by the Enterprise at the time. In the past, McCoy was seriously affecting the time line, so Kirk and Spock had to follow him to restore the shape of history.
Contracted the strange 'aging virus' that affected the Gamma Hydra IV landing party.

After having retired from STarfleet for a second time, he was forced back into service by Admirals Kirk and Nogura when V'Ger threatened the Earth.

Still active with Starfleet in his later years after retirement (for the third and final time), he inspeced the medical facilities on the new Galaxy class starship Enterprise at the age of 137.
