Originally dedicated to curing the torments of the insane, the gifted surgeon Dr. Channard discovered the Lament Configurations early in his researches. Not long thereafter, the Doctor's conscience was lost to Obsession, to the desire to explore the source of all madness -- Hell itself.

Was it chance that led Kirsty to the Channard Institute? Through Kirsty, Channard finally finds Hell's portal, and he sacrifices a patient to resurrect Julia, Kirsty's bloodthirsty stepmother. Mated in murder, Channard and Julia give a Box to an autistic girl named Tiffany, and as this young savant solves the puzzle, the gates to a dark dimension are thrown wide.

Once in the Labyrinth, Channard learns the truth -- he is more than another victim of Leviathan's appetite. He is transformed into a Cenobite, his face split with wires, serpents bursting from his palms, his bulk carried through Hell by the clawed tentacle of the Nether God. Now the pawn of Leviathan, he is sent to destroy the others Cenobites for the Sin of Remembrance.

Even in Hell the innocent sometimes prevail, and as Tiffany succeeds in closing the Box, Channard learns, to the melody of cracking cranial bones and his own screams, the true meaning of Hell: All will be sacrificed!
