§               Without advanced stage and without poor prognostic factors

§               70% cure

 

§            Low grade

           Chemo sensitivity fair but used and  

           or radiation

          May convert to high grade

§            Intermediate grade

        Chemo sensitive

        Radiation

         87% remission 73% survival

§            Patient with poor prognostic factors

        > 65 years of age

        Very high LDH

        Burkitt’s

        Marrow, GIT, CNS involved

        40% remission 26% survival

 

MOPP - increased incidence of sterility and secondary neoplasm

ABVD - best in younger patients

Both have similar response

M echlorethamine  IV  Day 1 & 8 MR q28d x 6 cycles

O vincristine          IV Day 1 & 8 MR q28d x 6 cycles

P rocarbazine         PO Day 1-14 MR q28d x 6 cycles

P rednisone            PO Day 1-14 MR q28d x 6 cycles

                    or

A doxorubicin  IV Day 1 and 15 MR q28d x 6 cycles

B leomycin       IV Day 1 and 15 MR q28d x 6 cycles

V inblastine       IV Day 1 and 15 MR q28d x 6 cycles

D ac  D acarbazine    IV Day 1 and 15 MR q28d x 6 cycles

CHOP

CHOP + Rituximab (R-CHOP)

 

 

C yclophosphamide  IV day 1 MR q21d x 6

H hydroxydaunorubicin - doxorubicin    

                                 IV day 1 MR q21d x 6 

O vincristine              IV day 1 MR q21d x 6 

P rednisone               PO day 1-5 MR q21d x 6
