Applications and requests to Aesthet Web require a nonrefundable fee of $55 in U.S. funds. No application will be processed unless the fee is paid in full or a fee waiver is granted. An international money order or check drawn through a U.S. bank made payable to and containing your company and name in full must be stapled to your application form/request. Do not send cash or coupons.
